PRACTICE OF LOOP ELECTROSURGICAL EXCISION PROCEDURE IN THE MANAGEMENT OF PREINVASIVE CARCINOMA OF CERVIX

The research was carried out in VIA and Colposcopy Center of Khulna Medical College Hospital from January 2014 to December 2015. Loop electrosurgical excision procedure (LEEP) practices a wire loop heated by electrical energy to eliminate cells and tissue in lower genital tract of a woman. It is used as part of the diagnosis and treatment for abnormal or precancerous lesions. This study aimed to assess and to protect women at risk from developing cervical cancer and to evaluate the advantages and pitfalls of the loop electrosurgical excision procedure as followed to the identification and management of cervical cancer precursors. Loop electrosurgical excision procedure applies a thin wire loop which performances as a scalpel. An electric current is conducted over the loop that slices a thin layer of the surface cells. Colposcopy is use to magnify the cervix during the surgery. The loop is injected over the vagina to the cervix. Various sizes and forms of loops may be used. An extraordinary cream can be used to cervix to halt any bleeding subsequently the procedure. A review of our initial experience, within two years a total of 12,287 patients were done undergoing routine visual inspection of cervix by application of acetic acid. Among them 486 patients (3.96%) were VIA positive. Among the VIA positive cases a total of 196 patients (40.33%) were managed by Loop Electrosurgical Excision Procedure (LEEP). Among them 98 patients (50%) were CIN I, 75 patients (38%) were CIN II and 23 patients (12%) were CIN III. Loop electro excision characterizes for identifying and curing cervical cancer precursors. Practice of authentic technique depends on the tissue essentials to be eradicated and where the cells are positioned on the cervix. Regaining time usual takes several weeks. As the techniques of LEEP have certain hazards consequently it is essential to recognize them before having the technique. Management of CIN by LEEP is reasonable, harmless and realistic.
